Application Process

1. Download the Application Form. (Requires Adobe Acrobat Reader)

  • Complete, sign and date the form.

2. Attach Center/Church Information as follows:

  • Resume of the Senior Minister/Director
  • Brief history of the Center, including beginning date, legal status (501c3, etc.), attendance, mission statement and other pertinent information
  • Statement of financial status (such as if adequately sustained by membership, volunteers, outside donors, etc.)

3. Attach Educational Programs specifying:

  • Instructional Programs: classes offered, average attendance, number of instructors, etc.
  • Educational Design: goals, mission and vision
  • Resources Available: financial, physical and technological.
  • Leadership and Governance: how decisions are made
  • Community Alliances

 4. Attach Application Fee of $250
