1. Download the
Application Form.

Complete, sign and date the form.
2. Attach Center/Church Information as
follows:
- Resume of the
Senior Minister/Director
- Brief history
of the Center, including beginning date,
legal status (501c3, etc.), attendance,
mission statement and other pertinent
information
- Statement of
financial status (such as if adequately
sustained by membership, volunteers,
outside donors, etc.)
3. Attach Educational Programs specifying:
- Instructional
Programs: classes offered, average
attendance, number of instructors, etc.
- Educational
Design: goals, mission and vision
- Resources
Available: financial, physical and
technological.
- Leadership and
Governance: how decisions are made
- Community
Alliances

Attach Application Fee of $250
5. Mail to the address on the form.
